Analytic Reviews High-Frequency Oscillatory Ventilation (HFOV) and Airway Pressure Release Ventilation (APRV): A Practical Guide

Despite advances in ventilator management, 31% to 38% of patients with adult respiratory distress syndrome (ARDS) will die, some from progressive respiratory failure. Inability to adequately oxygenate patients with severe ARDS has prompted extensive efforts to identify what are now known as alternative modes of ventilation including high-frequency oscillatory ventilation and airway pressure rel...

New modalities of mechanical ventilation: high-frequency oscillatory ventilation and airway pressure release ventilation.

Management of acute lung injury (ALI) and the acute respiratory distress syndrome (ARDS) is largely supportive, with the use of mechanical ventilation being a central feature. Recent advances in the understanding of ALI/ARDS and mechanical ventilation have revealed that lung-protective ventilation strategies may attenuate ventilator-associated lung injury and improve patient morbidity/mortality...

The 30-year evolution of airway pressure release ventilation (APRV)

Airway pressure release ventilation (APRV) was first described in 1987 and defined as continuous positive airway pressure (CPAP) with a brief release while allowing the patient to spontaneously breathe throughout the respiratory cycle. The current understanding of the optimal strategy to minimize ventilator-induced lung injury is to "open the lung and keep it open". APRV should be ideal for thi...

Pulmonary Fibrosis can occur in survivors of pediatric cancer. High frequency oscillatory ventilation (HFOV) and air- way pressure release ventilation (APRV) may be useful in the critical care management of a pediatric patient with Pulmonary Fibrosis

In this report we describe difficult ventilation during anesthesia as well as in the pediatric intensive care unit in a pediatric patient with pulmonary fibrosis. We discuss how an initial diagnosis of acute spontaneous pneumothorax, a correctable problem, misdirected perioperative planning and anesthetic management of a patient with severe pulmonary fibrosis.
